CAP Case Manager - Hybrid Remote

Job Type: Full-time, Hourly
Pay Range: Starting Pay, $18.75 - $20.00/hr.

Position Summary

Healthkeeperz is hiring a CAP Case Manager to support the Community Alternatives Program (CAP) for Disabled Adults and Children. This role provides assessment, care planning, coordination, and ongoing monitoring to help individuals safely remain and receive care in their homes.

Key Responsibilities

  • Conduct assessments and determine CAP eligibility
  • Develop individualized care plans based on identified needs
  • Coordinate services with families, providers, and community agencies
  • Perform home visits and follow-up monitoring
  • Maintain accurate documentation in E-CAP; review aide notes; support compliant billing
  • Ensure compliance with CAP and state program guidelines
  • Participate in required training and continuing education

Hybrid Work & Travel

  • Based at the Durham office
  • Eligible for up to 2 remote workdays per week after onboarding and supervisor approval
  • Regular home visits across Durham County and the surrounding counties (private homes and occasional facilities)
  • Flexible scheduling within program requirements
  • Must have reliable transportation; mileage reimbursement provided
Requirements:

Education & Experience: Applicants must meet one of the following:

  • Bachelor's in Social Work + 1 year related experience
  • Bachelor's in Human Services + 2 years related experience
  • Bachelor's in Non-Human Services + 2 years related experience
  • Current NC RN license + 1 year case management experience
  • Apprentice/Trainee (Bachelor's or RN without full experience) - requires ongoing supervision

Required Skills

  • Knowledge of casework, care coordination, and social services practices
  • Familiarity with community resources
  • Strong communication, time management, and organization
  • Ability to travel for visits and meetings
  • Proficiency with electronic documentation and virtual communication tools

Work Environment

  • Frequent travel in the community and varied home environments
  • Independent fieldwork with occasional exposure to unpredictable conditions
