The Health and Welfare Council of Long Island (HWCLI) is a private, not for profit, health and human services planning, research/public education and advocacy organization that serves as the umbrella for public and non-profit agencies serving Long Island's poor and vulnerable individuals and families.

The Health Equity Alliance of Long Island (HEALI) is Long Island's Social Care Network (SCN) of community-based organizations (CBOs) and healthcare providers building a person centric model that integrates healthcare, social care, and behavioral health care. HWCLI is the lead entity for the HEALI Social Care Network.

JOB ANNOUNCEMENT: HEALI Senior Director of Clinical Services

Health and Welfare Council of Long Island seeks a Senior Director of Clinical Services for the HEALI Social Care Network who can develop the value propositions, policies, procedures and oversight necessary to enable our SCN to successfully partner with health care providers and payers along our various program and service lines to advance integrative care. The ideal candidate for this position is an Advanced Nurse Practitioner with a keen understanding of and empathy for Long Island's communities and healthcare disparities as well as entrepreneurialism and business savvy. The successful applicant will work closely with the Executive Vice President and other members of the senior team to advance the strategic objectives of our organization.

Responsibilities include:

  • Providing direct or indirect medical oversight for all utilization review and care management and service coordination programs where such supervision is required by contract, law or regulation.
  • Identifying business opportunities (disease prevention and other evidence-based programs) with heath care providers including Federally Qualified Health Centers, patient-centered medical homes, and specialty medical groups, as well as managed care organizations in the marketplace including Medicare Advantage, Medicaid and dual eligible plans, commercial insurers, large self-funded employers, and state-owned plans.
  • Delineating and reviewing care outcome and impact reports and using this information to improve quality of social services provided, as well as to focus HEALI's resources on areas of greatest impact.
  • Facilitating access and utilizing outcomes data and validating the process and progress made during service delivery, early identification of status changes and intervention.
  • Helping manage health care relationships, challenges with clients and providing expertise and guidance when concerns of care are raised.
  • Involvement with network delivery including engaging health plan and healthcare partners in the impact of
  • Conducting team education on the importance of community integrative care with internal CHWs and across the HEALI network.
  • Assisting the HEALI team with creative solutions to barriers involving medical care, diagnosis, treatment and access issues, as well as identifying opportunities for improving Long-Term Services and Supports and Social Determinants of Health.
  • Representing the organization on medical issues at national and state-level meetings, including Department of Health, Office of the Aging, Department of Mental Health, public health associations, hospital associations, medical societies, and other professional meetings.
  • Assisting with public relations and advocacy efforts that promote the evidence-based programs of the organization within the community.
  • Developing initiatives related to building community partnerships and capacity for services provided to health care systems members, and engaging HEALI's senior leaders in promoting the organization, including through participation on health care-related boards, councils and other policy-making organizations.
  • Staying abreast of developments in the health care environment locally, regionally and nationally to help guide HEALI's core mission and strategy.

Qualifications, Required Skills and Experience:

  • Advanced Nurse Practitioner with active New York license
  • Primary care specialty preferred
  • Knowledge of Value Based Payment landscape
  • Knowledge of Medicaid and Medicare regulatory environment
  • Experience in analysis, management and evaluation of programmatic impact and ROI
  • Champion of social care with strong connections to existing health care provider leadership and professional societies
  • Experience utilizing social service programs and initiatives
  • Knowledge of the Long Island region, demographics and communities
  • Working knowledge of evidence-based programs
  • Experience with advocacy efforts to promote community programs
  • Experience with marketing, including multimedia presentation development
  • Strong oral and written communication skills
  • Strong interpersonal skills, including the demonstrated ability to work effectively with members of the senior team and staff
  • Keen understanding of and empathy for HEALI's primary client populations
  • Proficiency in Microsoft Office programs, including Outlook, SharePoint, Word, Excel and PowerPoint
  • The Medical Director reports to the Executive Vice President.
